Jagger Smith is a member of the Senior Executive Team at Baycrest Hospital. He is the administrative Director for Ambulatory Clinical Services. Jagger actively contributes to two Ontario Health Teams: North Toronto Ontario Health Team and North York Toronto Health Partners. Jagger leads specialist clinics, interprofessional outreach teams, and integrated care models that support an estimated 400 older people – who are eligible for Long Term Care – to age-in-place, in the community, every day. While completing his Masters of Health Administration, and getting married, Jagger lived in a Centertown Citizens of Ottawa building.
